Date:______ Puppy-male/female ___w/out AKC or CKC registration papers___with AKC or CKC registration papers to be received within
180 days from date or purchase by mail(please make sure to include your mailing address at the bottom-please print clearly). Breed:

Your puppy has been wormed & received all age appropriate vaccinations(health record is included in your packet). You must have your
new puppy examined by your veterinarian within 3 buisiness days of purchase. If you fail to do so, this contract becomes void.
We give a limited health guarantee of our puppies for 3 months from the time of sale to be free & clear of any congenital, life threatening
illnesses or diseases (does not include hernias, pinched nostrils, open fontanels or retained testicles).
Great care has been excercised to control & eliminate parasites and/or skin conditions, such as intestinal worms, coccidiosis, fleas, mites,
giardia, kennel cough ,etc..Since these conditions are somewhat common, we will not guarantee this puppy to be totally parasite free.
Because these conditions are easliy treated, they are not conditions that will constitute a replacement of the puppy. Only congenital, life
threatening illnesses or diseases will be covered under contract.
In the event that this occurs, upon proof of a veterinarian diagnosis plus a second opinion from a vet of our choosing(we do not pay vet bills
acquired), we will replace your puppy of the same breed of equal or lesser value when available and at that time you may return your puppy
to us.. (sorry, no cash refunds). If you should lose your puppy due to genetic illness(accidents or neglect are not included) within 3 months
from the date of purchase we will replace your puppy (when available). A Veterinarian statement regarding cause of death will be required.
To qualify under this provision, you must keep the puppy under the regular care of a licensed Veterinarian. This care must include the initial
veterinarian examination within 3 days of purchase, a complete immunization and worming program (this is to include monthly heart worm
protection) and immediate treatment for any signs of illness or injury. You will be required to provide written documentation of complete vet
records on the puppy. We do not guarantee size , color or breeding ability of any puppies we sell..
It is our recommendation that you have your puppy spayed/neutered at the appropriate age (talk to your Vet). If you neglect to do any of
the above, this contract becomes void. This guarantee is limited to the original purchaser only. If the pup is resold, given away or transfers
ownership, guarantee is VOID.
THE SELLER IS NOT RESPONSIBLE FOR ANY VETERINARIAN COSTS INCURRED BY THE DOG AND IT’S OWNER AFTER PURCHASE.
